The individual plan aims to identify the level of support needs (high, moderate, low) of a student with an identified disability irrespective of educational setting. A profile of a student is developed across 6 focus areas: curriculum participation, communication, mobility, personal care, safety and social skills, by identifying the adjustments that are currently in place to support the student and using the examples of descriptors to assist with the identification of a level of support for each focus area.
